Appeal Form

This form is intended to be used to appeal an academic action (e.g. probation, suspension, policy violation, etc.) All appeals are reviewed by the Dean of Academics and, when appropriate, the Academic Review Board. Please provide clear, concise, and specific information regarding the nature of appeal and what resolution you seek.
