Outside Employment Disclosure Form
Oak Valley College requires exempt full-time employees to disclose outside employment or significant business/nonprofit activities to ensure that such activities do not create or appear to create a conflict of interest or commitment, or interfere with your responsibilities to the College. All conflicts or potential conflicts are subject to review by the Board Executive Committee of Oak Valley College.
Not all conflicts are necessarily forbidden. For instance, an employee may choose to serve on an elder board, which meets Monday afternoons (a conflict with regular work hours). An employee may be granted permission to serve as an elder with supervisory approval. Permission is not automatic.
Failure to divulge a conflict or failure to seek approval to a conflict, may be grounds for disciplinary action, up to and including termination.
Outside Employment/Activity Information
Please disclose ALL PAID outside employment, self-employment, or consulting.
In addition, you are asked to divulge:
business interests that may conflict with your position at Oak Valley (e.g. interest in a vendor or prospective vendor)
volunteer or church roles with substantive regular meetings and responsibilities (e.g. nonprofit board member, church elder, hospital chaplain)
other outside activities that may create regular conflict with your work schedule, priorities, or responsibilities
FOR MULTIPLE DISCLOSURES, PLEASE SUBMIT A SEPARATE FORM FOR EACH ORGANIZATION AND ROLE
YOU DO NOT NEED TO DIVULGE CASUAL, TEMPORARY, UNPAID ROLES THAT DO NOT PRESENT A REASONABLE APPEARANCE OF CONFLICT (E.G. SALVATION ARMY GALA VOLUNTEER, SUNDAY SCHOOL TEACHER, OR BIBLE STUDY LEADER)
